bankruptcy forms instructions

I need help with bankruptcy schedule F.?
I am filling out bankruptcy forms, although there were hints to the shape my question is on schedule F. If Are there an original creditor and then the account to another agency for the collection gets sent, now has 2 records in the same debt, I put both on the form or just the original? If you do this before I could use your input to possibly speak more than just the answer to this question. Thank you for Your help.
